The Appointment
At the appointment you will be required to provide the medical history forms, pay the fee and provide a valid California ID.
For New Patients
Please print and fill out the Release of Medical Records Form first, and send to your doctor. After we receive your medical records from your doctor, we will then call you, and at that time have you fill out the New Patient Appointment Packet.
